Classification
Core: Sport-Related Concussion Acute (time of injury until 72 hours) and Subacute (after 72 hours to 3 months)  Should use oneof the symptom checklists.
 
Supplemental–Highly Recommended: Sport-Related Concussion (SRC) - Persistent/Chronic (3 months and greater post concussion)
 
Supplemental: Traumatic Brain Injury (TBI)
Short Description of Instrument
An objective and standardized method of immediately assessing an injured athlete's mental status on the sport sideline within minutes of having sustained a concussion. The assessment in intended as a supplement to other assessment measures and the medical provider's clinical examination.
Scoring
Total score can range from 0 to 30 resulting from performance on each of the following domains: Orientation (5pts), Immediate Memory (15pts), Concentration (5pts), and Delayed Recall (5pts)
